Atherosclerotic Dementia with Epilepsy

Abstract
The narrowing and occlusion of the cerebral vessels by atherosclerosis are important factors in the production of cerebral manifestations, especially when the cerebral blood circulation is impaired. The symptoms and signs resulted from atherosclerosis largely depend upon the site of the lesion and the extent of functioning collateral circulation.
A case of atherosclerotic dementia whose initial symptoms started with an epileptic seizure and who responded well to a combined treatment with Dilantin and Jacobson¢¥s solution with vitamin A is clinically observed. During the first six weeks the patient had been ¢¥treated with only anticonvulsant drug (Dilantin, 0 1 tid", and tranquilizer (Chlorpromazine, 25 mg. tid). The epileptic attacks were well controlled although the mental deterioration became steadily worse. The patient finally suffered from marked degree of disorientation, memory deficit and slight degree of confabulation, delusion, motor dysphasia and occasional visual hallucination.
Approximately ten days after the use of the solution of Benzyl Cinnamate with vitamin A (Icc. daily IM Injection), the patient¢¥s mental symptoms began to subside and considerable improvement resulted in three weeks.
Marked satisfactory changes were seen in the retinal vessels fundoscopically following the treatment with Jacobson¢¥s solution; more abundant vascularization and less tortuosity in vessels were remarkable. Considerable effect on cortical function was observed in electroencephalograms; irregular slow wave (3 ,cps.) noted in the right frontal area in previous EEG, disappeared completely. No comparable cerebral angiogram or pneumoencephalogram were obtained.
During or after the use of Jacobson¢¥s solution with vitamin A, no side effect was observed. .
The author believe the solution of Benzyl Cinnamate with vitamin A has effect on ¢¥ the formation of revascularization and thus improving the function of collateral circulation leading satisfactory cell metabolism in the atherosclerotic brain.
Further clinical investigation with this drug will be followed.
